Below is the medical release form for 2010. This form is valid for all HSM and MSM events for 2010.  If you haven't yet filled one out, download it and bring it on in. These forms are required for all out-of-town events, especially the missions trip and retreats. We will put it on file and it will be good for the remainder of the year.
